How do you earn rewards?

You can earn rewards for completing preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.

 

There is no extra cost for preventive care. Better yet, we reward you for taking steps toward health.

Earn gift cards and more

You can earn rewards like gift cards and other items for each healthy step you complete. So it really does pay to check on your health.

Wellness visits

 

  • Earn $25: Complete yearly well care visit (ages 12 to 21 years)
  • Earn $50: Complete follow-up visit after emergency room visit for mental health within 7 days or $25 for follow-up within 30 days (ages 6 and older)
  • Earn $50: Complete follow-up visit for discharge for treatment of substance use within 7 days or $25 for follow-up within 30 days (ages 13 and older)

Vaccines and lead screening 

 
  • Earn $50: Flu Vaccine Incentive –Complete 2 flu shots before age 2
  • Earn $50: Human Papillomavirus (HPV) Vaccine Incentive – Complete HPV shots before age 13
  • Earn $25: Complete lead screening blood test before age 2
  • Earn $25 WV State Park Gift Card: Receive annual flu shot (adult members 18 and older)

Diabetes management 

 

  • Earn $25: Enroll in a diabetes support program and complete A1C testing
  • Earn $50: Complete yearly diabetic eye exam (ages 18 and older)

Healthy actions

 

  • Earn $25: Complete a walking program (age 18 and up)

Pregnancy and birth care

 

  • A cribette or baby carrier: Enroll in the Moms and Babies program
  • A cribette: Complete six prenatal visits with your provider
  • Earn $50: Complete one postpartum visit with your health care provider (within 7 to 84 days after birth)
  • A toy for your baby’s first birthday: Complete the Moms and Babies program

School and learning

 

  • $50 toward afterschool programs: Join an afterschool program; must use the reward to pay toward membership fees (join a 4H club and Aetna Better Health will cover program fees)
  • Camp Scholarships: Enroll in a children’s health related summer camp

Care and safety

 

  • Earn $25: Complete an ATV rider safety and training course
  • Medication lockbox: Available to secure narcotic medications
  • Dental Hygiene Kit: Yearly incentive includes toothbrush, toothpaste, and floss.
  • Asthma Management: enroll with a CM, eligible for a peakflow meter to help with breathing exercises.
  • CHF Incentive: upon CM enrollment of CHF, one time bathroom scale and weight log.

Breast Cancer screening

 

  • Earn $50: Complete mammogram (ages 50-74)

Cervical Cancer Screening

 

  • Earn $50: Complete cervical cancer screening/pap test (ages 21-64)

Ted E. Bear, M.D., Cub Club

 

Members 13 years of age and under to encourage healthy habits and lifestyle. With the Ted E. Bear, M.D. Cub Club, you can earn rewards for healthy activity challenges, such as:
